My health policy number is 441003/48/2018/714 ans validity till 5/6/2018. Original policy was taken on 5/6/2017 and subsequently renewed on 5/6/2017. Now again issued cheque for renewal through PNB Mankavu Branch for Rs.14116/- (for coverage of 3 lakhs). I have submitted a claim for hospitalization of mu Daughter Miss C.P.Suja through The oriental Insurance Co. Kings way building, Mavoor road, Calicut on 9/5/2018.
